Appointment Information

New patients need to have a referring physician call to schedule their first appointment with the clinic. Once contacted by your physician, please call our office to confirm your appointment.

To make a returning appointment, please call (704) 403-1308 and select prompt #1. In the event our staff is unable to answer your call, please leave a message and your call will be returned as soon as possible.

First Visit
Each new patient is asked to print and complete the new patient forms.

Please bring your insurance card and any prescription medications to your appointment. Each new patient is asked to arrive 30 minutes early to complete registration and other paperwork. Returning patients are requested to arrive 15 minutes early so we may verify and update any information as needed.

Emergencies
Our physicians are on-call 24 hours a day to offer help to patients in need. If you have an urgent problem, after hours or on a holiday, please call CMC - NorthEast at (704) 789-2144.

Cancellation Policy
Please call to cancel any appointments you will be unable to keep at least 24 hours in advance so we may offer these appointments to other patients in need of treatment.
