Q. What do I need to bring with me to my appointment?
A. For new patients, please print and complete the new patient forms from our website. For ALL patient visits, please bring your current insurance or Medicaid cards, any prescription medication bottles, plus any over the counter medicines you are currently taking.

Q. Can I pay my past due balance at my appointment?
A. Yes, we accept cash, personal checks, Visa, MasterCard and Discover cards.

Q. How early do I need to arrive for my appointment?
A. Each new patient is asked to arrive 30 minutes early to complete registration and other paperwork. Returning patients are requested to arrive 15 minutes early so we may verify and update any information as needed.
